Well I am afraid I can't be of much help but I do have an observation.
You also get the above fatal error message if you try to use a
tear-off menu.

I'm not exactly sure when it broke, but I am fairly certain that it
was working in 3.1.x.

So my suggest would be to maybe do some diffing between 3.1.x and
3.99.4 to help track down the source of the problem.

I made a half-hearted attempt to discover the source of the tear-off
problem, and gave up (too) quickly...
